What to Expect from your Care Coordinator

Create Your Personal Health Notebook
We recommend organizing your basic health information in a designated notebook. This will help to prepare you for the first follow-up call with your Care Coordinator after your initial enrollment.
Suggested Information Includes:
- Medications (including dosage and timing)
- Recent and upcoming medical appointments
- Recent lab/test results
- Current diet and exercise patterns
- Sleep schedule
- New or worsening symptoms
- Barriers that make it difficult for you to eat well and/or stay active
- Questions or concerns

Staying Engaged with Your Care Coordinator
- Make sure to save your Care Coordinator's number and call them back if you miss their call. Leave a message so that you can be sure to connect with them every month.
- Add nurses@mycarecoordinator.com to your email list to ensure that you are receiving MyCare Journey content.
- Review the content you are sent weekly and discuss it with your Care Coordinator if you have any questions.
- Get to know your Care Coordinator and utilize them as a resource while navigating your journey to improved health.

Scale Troubleshooting
If you're having trouble with your scale, please try the following:
- Make sure your scale is on a hard, level surface.
- Connectivity issues? Try moving your scale to an exterior wall.
- Be careful not to get your scale wet.
- Still having trouble with your scale? Call 866-256-1951 for assistance.

Scale Tips
- Position your scale in a place where you'll be reminded to step on it every day.
- Set a reminder in your phone to weigh in every day.
- Still having a hard time remembering to step on your scale daily? Try tying your weigh-in to an existing habit (i.e. stepping on your scale every time you brush your teeth).

Why haven't I received my scale yet?
If you've enrolled in our MyCare Device service, you will be eligible to receive your scale after you've completed two calls with your Care Coordinator (not including your enrollment call). If you have specific questions about when your scale will be delivered, feel free to ask your Care Coordinator.

How do I access MyCare Journey Content?

MyCare Journey content will be sent to you on Monday, Wednesday, and Friday each week.

The content will be sent either via email or text. We strongly encourage you to provide an email address as it allows us to provide you with a better experience throughout the journey. If you haven't already, share your email address with your Care Coordinator during your next call!

How can I access MyCare Journey content you sent previously?
MyCare Journey content that has been sent to you previously will be available for you to review in our lookback logs. Lookback logs are included in the resource section every Friday. The lookback logs will also allow you to preview which topics you'll be learning about in the upcoming week.

How do I play videos?
If you receive text messages:

Click the link in the text message which will take you to a website page

When the website page opens, you will see a title describing what the video is about. Underneath the title will be an image from the video with a "play" button over top (triangle with a circle around it). Click anywhere on the image to play the video.

You can pause the video by clicking anywhere on the video at any time. If you want to begin playing the video again, simply click anywhere on the video.
If you receive email messages:

When you open your email, you will see a button with an arrow pointing towards it that says "click here to watch". Click this button which will bring you to a website page. If you're having trouble getting the page to open, try clicking the "view in browser" button in the upper right-hand corner first, then click the "click here to watch" button.

How do I log in to play games?

To play games, you will click on the "click here to play" button in the email we send you on Fridays. This will bring you to a website page.

On the website page, you will see a header describing which game is available that week. Beneath the header will be a picture of either Jeopardy, Wheel of Fortune, or Trivia. This image will have a "start" button.

Click the "start" button. You might see a brief message describing what the game is about. If so, click "ok".

You will then be brought to the registration page. You will need to input your first name, last name, and email address. Please note email addresses are required to play these games and we ask that you please use the same email address we send your MyCare Journey content to when registering.

Once you put in your information, hit "register" and your game will begin.

How many times can I watch videos or play games?
You can watch MyCare Journey videos and play MyCare Journey games as many times as you'd like. In fact, we encourage you to watch our videos and play the games more than once!

Are there prizes awarded for playing MyCare Journey games?
No, MyCare Journey games are just for fun and do not include prizes. Certain games, like Jeopardy, will show how much "money" you're earning throughout the game, but this is solely for entertainment purposes and no cash or other prizes are awarded for playing these games.
